Please print form, fill out and email to Events@CharityGuild.org.

Step 2

Please do NOT pay Booth/Table fee until you have submitted your Application AND have received email confirmation that you have been accepted to participate in the Event.

Mail to: Country Club of the South, 4100 Old Alabama Road, Johns Creek, GA 30022. Table/Booth confirmed upon receipt of BOTH Payment & Application.
